Care navigators work with your GP providing non-medical support to help you take control of your health
How do they help?
A Care Navigator otherwise known as a Social Prescriber can link you to:
- Housing and benefits advice
- Information about social care
- Befriending and counselling
- Healthy lifestyle advice
- Support groups
- Mental Health Support worker
- Clinical Pharmacists/Technicians
As part of the service our Receptionists also form part of the Care Navigators and in order to sign post you to the most appropriate clinician your call will be triaged by the Receptionist.
